Summary:
Under the supervision of Director of Case Management, the social worker will assist in meeting the psycho social needs of patients in order to achieve optimal health outcomes. Assess patients for discharge needs and assists with making appropriate referrals for local|state|community resources. Educates family, patient and staff on services beyond the hospital homecare, hospice, DME, VA etc. Assists with filing applications, and financial applications, Handles adoption issues on Womens Care as requested. Works closely with victims of abuse and neglect for support and appropriate referral. Makes appropriate referrals to department of Human Services. Assists with guardianship as requested. Area of primary focus will be the emergency and complex Case Management.
Education:
Graduate of an accredited School of Social Work, require
Masters Degree in Social Work, required.
Experience:
Psychiatric Social Work on a fast paced inpatient psychiatric unit, preferred.
Certifications | Licensure:
Current LSW or LCSW in Pennsylvania
