Job Description

Our skilled nursing facility is looking for a full-time Admissions Clinical Liaison near Andover, MA . The Admissions Clinical Liaison plays a pivotal role in facilitating the admission process for individuals seeking long-term care or rehabilitation services.

Responsibilities for Admissions Clinical Liaison

Assessment and Evaluation:

  • Meet with prospective residents and their families to assess their medical needs, personal preferences, and care requirements.
  • Gather relevant medical history and documentation to evaluate eligibility for admission.

Information Provision:

  • Educate prospective residents and their families about the services, amenities, and policies of the nursing home.
  • Address questions, concerns, and provide detailed information to assist in decision-making.

Tours and Presentations:

  • Conduct tours of the facility to showcase living spaces, amenities, and care services.
  • Present information about the nursing home's programs, activities, and staff qualifications.

Admissions Coordination:

  • Guide families through the admissions process, including completion of paperwork, medical assessments, and financial arrangements.
  • Coordinate communication between healthcare professionals, social workers, and the nursing home to ensure a seamless transition.

Documentation and Record Keeping:

  • Maintain accurate records of all admissions-related activities, including assessments, communications, and documentation.
  • Ensure compliance with regulatory requirements and internal policies.

Follow-up and Support:

  • Conduct follow-up with residents and families after admission to address any concerns and ensure satisfaction with services.
  • Provide ongoing support and assistance as needed throughout the residency period.

Qualifications and Skills for Admissions Clinical Liaison

  • Bachelor's degree in a relevant field such as healthcare administration, social work, nursing, or a related discipline preferred.
  • Clinical experience in a skilled nursing facility setting preferred.
  • Strong communication skills, both verbal and written
  • Familiarity with post-acute care, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred.
